Please provide complete insurance information at the time of scheduling or registration. This information should include a copy of your insurance card(s), driver's license or picture identification. We also ask that you inform Martha Jefferson Hospital registration staff of any changes or updates to your personal information if you have previously had services at Martha Jefferson Hospital.

Please comply with all requirements and respond timely to your insurance company regarding referrals, authorization of services and completing coordination of benefits forms. For questions regarding your insurance coverage, please refer to your policy plan booklet or call your insurance company directly.

Martha Jefferson Hospital will collect per visit co-payments at time of service in compliance with a patient's insurance benefit terms and will file insurance claim forms for most insurance plans when assignment of benefit has been provided by the patient at the time of registration. The patient or responsible party may be asked to assist in working with Martha Jefferson Hospital and their insurance company if the insurance company fails to pay a claim that meets all payer/billing requirements within 40 days of claim receipt in accordance with Virginia law.

Physicians affiliated with Martha Jefferson Hospital are private practitioners. As such, their participation with insurance plans may vary and do not necessarily mirror the hospital's participation.
